Population Health has achieved a significant level of interest over the past few years.  David Kindig published some early work in 2002  See "What is Population Health?" in the American Journal of Public Health.

The Triple Aim achieved even greater visibility when it was included in the Triple AIm by Donald M. Berwick, Thomas W. Nolan and John Whittington.  and first published in Health Affairs.   Don Berwick, the founder of the Institute for Healthcare Improvement went on to become the CMS Admistrator for the Obama Administration. CMS adopted the Triple Aim as one of its key initiatives
